その後、受付で診察費と薬代払ったんだけど3980円だった

And then, I paid the medical examination fee and a charge for medicine at hospital reception, the total was 3980 yen.
そして病院受付で診察料と薬代を支払い、合計3980円でした。

薬代

The medical examination fee is around 600 yen. The cost of the medicine varies depending on the day.

意外と安い…
しかもお薬付きで…

It was cheaper than I thought. Besides, the fee include the charge for medicine.
思ったより安かったです。 なお、料金にはお薬代も含まれております。

提携してる薬局があって普段はそっちに薬は取りに行ってもらってるみたいだけど、保険証ないからか院内処方って形で出してくれたみたい

Usually a patient would go to a pharmacy near the hospital, but I didn’t have national insurance card, so the hospital was considerate of me prescribing as in-hospital.
通常患者は病院近くの薬局に行くのですが、私は保険証を持っていなかったため、病院側の配慮で院内処方で処方してもらいました。

普段なら

彼は普段なら、今○○してるよ、とか連絡してくるのに、昨日はしてこなかった。
